How would you pay your bills if you got sick or injured and couldn’t work? Disability coverage can help replace some of your regular pay. Caleres offers two types of disability coverage: short-term and long-term. See the chart below to learn about each coverage type.

Benefit Benefits depend on where and how long you’ve worked for Caleres; see the Short Term Disability Plan Policy  and be sure to talk with HR Services or call 1-314-854-4261 60% of your pre-disability base monthly earnings, to a maximum of $20,000 per month*
Who’s eligible? See the Short Term Disability Plan Policy Any full-time associate is eligible. All new hires can enroll in LTD within their 30-day new hire window. Anyone else interested in enrolling in LTD outside of new hire window must complete EOI form and send the form to Reliance. Please note LTD coverage is not active until Reliance approves EOI form.
Who pays for this benefit? Caleres You and Caleres share the cost
Do I have to enroll? No—this coverage is automatic Yes—any new hire can enroll within their 30-day new hire window. Anyone else interested in enrolling outside of new hire window must complete EOI form and send this form to Reliance for approval. Instructions are on the EOI form.
How do I file a claim? Download the Matrix eService Mobile app, go to matrixabsence.com or call 877-202-0055 Matrix will work with you to coordinate the LTD process if your disability extends beyond the STD period

How do I cancel coverage? You can't cancel STD coverage.

You can cancel LTD coverage anytime by going to Ceridian.

  • Go to the forms section
  • Complete the form that states 'Decrease or cancel voluntary life, AD&D and/or LTD coverage'
  • You will be notified once coverage is officially cancelled

*Your LTD benefit is reduced by Social Security and other disability benefits. If you’re disabled before age 60, these payments will continue until you’re no longer disabled or you reach age 65, whichever is earlier. If you’re age 60 or older when you become disabled, see the SPD for a detailed table on length of coverage.

Important Notes:

  • For work-related disability, see your Workers’ Compensation benefits.
  • Pregnancy and pregnancy-related conditions are eligible for Short-Term Disability benefits just like a disability. Find more info here.
  • Continued health insurance while on LTD is available through COBRA for 18 months. You may be able to get 11 more months of coverage if you provide evidence of a Social Security Disability Determination.
