Make changes to your benefits

Be sure to make any of the following benefit changes within 30 days of the event:

• Add or remove your spouse from health, dental and vision benefits. See if your spouse is eligible.
• Start, stop, increase, or decrease supplemental life insurance for you or your spouse.
• Change how much you're putting in your flexible spending accounts.

To request changes, go to Dayforce to complete the “Life Event Declaration Form” (Benefits > Forms):

• Provide the required supporting documentation for your qualified event within 30 days.
• You will receive a notification through Dayforce once your request has been approved or denied. If approved, you can make changes to your benefit elections.
• In most cases, any changes you make are effective as of the date of the life event.

Update your personal information

Change your address or tax withholding choices in Dayforce.

If you need to change your name, contact Dayforce at 1-866-279-4357 or Email via:MDFCaleres@ceridian.com. Please supply a copy of the marriage certificate and your Social Security card.

Update your beneficiary designations

Update your life, accidental death, or business travel accident insurance beneficiaries in Dayforce.

To update the beneficiary for your 401(k), log into Principal or call 1-800-547-7754.
