Find in-network care
No matter which plan you choose, staying in the UHC network saves you money. Search for in-network providers or call 1-888-298-1061.
Note for Surest members: Surest utilizes the UHC and Optum Behavioral Health networks, but some providers may not recognize the Surest name. Your medical ID card lists your payer ID and UHC network — provide this information to your doctor’s office. For more information, visit UHC for Surest.

Mental well-being support from Calm Health
Calm Health is available at no cost to you and your dependents age 16 or older if you’re covered by a UHC plan (now) or Surest (starting Jan. 1, 2025). It includes online and app-based resources for sleep, meditation and mindfulness. It also offers mental health screenings, self-guided learning, evidence-based content and referrals.
