New baby? New marriage? For each of life’s big moments, your benefits have your back.

Certain events in life may affect your benefits. If you experience a qualified life event, be sure to request benefit changes within 30 days of the event. If you miss the deadline, your current elections will remain until the next Open Enrollment.

To request changes, go to Dayforce to complete the “Life Event Declaration Form” (Benefits > Forms):

  • Provide the required supporting documentation for your qualified event within 30 days.
  • You will receive a notification through Dayforce once your request has been approved or denied. If approved, you can make changes to your benefit elections.
  • In most cases, any changes you make are effective as of the date of the life event.

Change in residence

If you’re moving, it’s important to consider if your new residence is outside of your plan’s coverage area. Be sure to let Caleres know your new address.
