Eligibility
You’re eligible for life and AD&D insurance through Caleres if you’re a regular full-time associate working at least 30 hours a week in one of these positions:
- Corporate office associate
- Distribution center associate
- Retail associate
- Assistant or associate retail manager
- Retail manager
Life and AD&D insurance
This coverage pays a cash benefit to your beneficiaries in the event of your death, or if you die or lose a limb, hearing, or eyesight from an accident.

Basic coverage
Caleres provides eligible associates $10,000 in basic life and AD&D insurance coverage at no cost.
Supplemental coverage
You can choose to buy additional coverage for you, your spouse or eligible domestic partner, and children as a new hire or during Open Enrollment. An employee must be insured for employee supplemental life to be eligible to enroll for spouse/domestic partner or child supplemental coverage.

Evidence of Insurability (EOI)
EOI may be required depending on what level of coverage you choose, when you enroll, and who you cover. If you enroll during your new hire event or a qualifying life event, you are eligible for the guaranteed issue amount, which means Evidence Of Insurability would not be required. The guaranteed issued amount for employee is the lesser of 3 times your salary or $500,000, and up to $50,000 for spouse/domestic partner.
After you complete your enrollment in Dayforce, log into Online EOI to complete EOI electronically with the following information:
- Policy Number: 70111
- Access Key: Caleres
EOI is not required if you:
- Enroll yourself, spouse or domestic partner, or children in supplemental AD&D insurance.
- Enroll in supplemental life insurance for yourself up to one time your base annual earnings as a new hire or within 31 days of a qualified life event.
- Increase existing supplemental life insurance for yourself by one time your base annual earnings up to three times your base annual earnings or $500,000, whichever is less.
- Increase existing supplemental life insurance for your spouse or domestic partner by one increment ($10,000) up to $50,000 within 31 days of a qualified life event.
Business Travel Accident Insurance
This coverage is provided at no cost to all active, full-time associates and pays a benefit if you’re injured or die in an accident while traveling on company business.
