Open Enrollment is November 7-18, 2022. This year, we encourage you to review which medical insurance plan provides the best value to you based on your lifestyle and your needs in 2023. Take time to review the following medical benefits information carefully so you can make the right decisions.

Caleres Medical Insurance Plan Objectives

We are committed to offering benefits that reflect your needs and expectations with the following objectives:

  • Provide choice – our 2,500 Caleres Associates have different needs, so we offer a choice of medical coverages.
  • Provide effectiveness – Caleres self-insures all components of our medical coverage which allows us to control plan design to best serve our Associates’ needs.
  • Remain competitive – we strive to remain competitive with our peer group for the value of our plans and the cost share between Caleres and our Associates.
  • Provide ways to maximize your benefits and save money – you can take advantage of preventive care at 100%, use network providers, pursue a healthy lifestyle, manage chronic conditions and participate in available programs, and communicate effectively with your healthcare providers.

Medical Rates Are Increasing in 2023

In recognition of turbulent times from 2019 –2022, including during COVID when maintaining consistency in health care was so important, Caleres chose to absorb the rise in all medical plan premium costs. This allowed our associate premium costs to remain flat for 4 years while medical costs rose 24% nationally.

Medical plan costs will rise again in 2023 and this year we must implement a medical premium rate increase:

  • For Associates hired before 2022, the average increase will be 14%
  • For Associates hired during 2022, the average increase will be 7% (an adjusted rate was already implemented in 2022)

Please note, deductible amounts will not increase. Inflationary pressures make predicting 2023 costs difficult so we may need to revisit this mid-year if costs are not better controlled.

New Medical Plan

Based on employee feedback, we have made changes. In 2023, Caleres is introducing a new medical plan option, the Surest Plan, by United Healthcare. Plan highlights include a zero deductible, no coinsurance, copays with negotiated rates and typically 100% coverage of preventive care. With the Surest Plan, you have more control, flexibility and opportunities to manage your healthcare decisions and budget.

Special Information for 50/50 Plan Participants

The 50/50 Plan will be discontinued as of December 31, 2022. If you are currently in the 50/50 Plan and want to continue medical insurance coverage through Caleres, you must choose another plan. If you do not make another medical plan election during Open Enrollment, you will not have coverage in 2023 and you will not be able to make a medical plan election until the next open enrollment period unless you have a qualified life status event.

Medical Plan Enhancements

As a result of feedback received from Caleres Associates, we have made multiple enhancements to the UHC plans and the Surest plan for 2023. Acupuncture is now a covered service, with a visit limit of 60 visits per year. Visit limits for Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy, Cardiac rehabilitation & Pulmonary rehabilitation have all been increased from 40 visits per year to 60 visits per year, allowing Caleres Associates more freedom when seeking conservative care.

NEW MetLife Voluntary Plans

Critical Illness and Accident Insurance will change from UNUM to MetLife with less cost and more coverage. Through MetLife’s Voluntary Plans, you can supplement your medical plan coverage and provide you and your family with the additional coverage and financial protection you may need for expenses associated with an unplanned covered accident, illness or hospitalization. This year, you can consider coverage under the following plans:

These plans pay benefits directly to you. What you do with the money is all up to you.

ENROLLMENT PROCESS

If you want to make changes to your current coverage, be sure to enroll during Open Enrollment, November 7-18, 2022. When you’re ready to enroll, go to Ceridian Dayforce to make your choices. Click here for detailed enrollment steps.

What happens if I don’t actively enroll in 2023 benefits?

  • Your healthcare coverage will remain the same in 2023, but your Flexible Spending Account (FSA) and Health Savings Account (HSA) contributions will not continue, as well as your participation in parking/transit FSAs. You must enroll in these plans if you wish to participate in 2023.
  • If you are currently in the 50/50 Plan and want to continue medical insurance coverage through Caleres, you must choose another plan.
  • You won’t have an opportunity to make changes until the next annual open enrollment period, unless you experience a qualified life event (for example, a birth, adoption, marriage or divorce).
IMPORTANT! If you experience a qualified life event and want to make changes to your benefits as a result, you must do so within 30 days of the event.

Garner Health for Consumer Plan Participants

If you elect the Consumer Plan, you and your covered family members can find high-quality doctors and healthcare providers with free resources from Garner Health. If eligible and you use Garner to find a doctor and see that doctor for medical care, Caleres will cover $1,000 of your eligible out-of-pocket medical bills ($2,000 for families) after you pay the first $1,400 of your deductible. This includes X-rays, imaging, surgeries and anything that’s covered by the Consumer Plan. Click here to learn more.

Good News – Voluntary Life Rates Are Decreasing

Special Opportunity to Increase Your Coverage

For 2023, you can purchase Voluntary Life Insurance at reduced rates. Consider the coverage you need based on your financial responsibilities and talk to a financial advisor to help you determine what you need. You may enroll for the first time, or increase employee coverage by 1x salary (up to a maximum of 3x your salary or $500,000, whichever is less) without providing evidence of insurability.
