Change in Employment

If you or your spouse has a change in employment status, you may have some important decisions to make.

Here are some examples of changes in employment status:

  • New job
  • Loss of job
  • Change from temporary or part-time to full-time, or vice versa
  • Retirement

If you are leaving or retiring from Caleres be sure to review the Leaving the Company FAQs.

Make changes to your benefits

Be sure to make any of the following benefit changes within 30 days of the event:

  • Add or remove you or your dependents from Caleres’ health, dental and vision benefits. See what your dependents are eligible for.
  • Start, stop, increase, or decrease supplemental life insurance for you or your spouse.
  • Change how much you're putting in your flexible spending accounts.

To request changes, go to Dayforce to complete the “Life Event Declaration Form” (Benefits > Forms):

  • Provide the required supporting documentation for your qualified event within 30 days.
  • You will receive a notification through Dayforce once your request has been approved or denied. If approved, you can make changes to your benefit elections.
  • In most cases, any changes you make are effective as of the date of the life event.

Notes:

  • Your coverage and contributions will end on your last day with Caleres. You may be able to continue coverage under COBRA. You also may have deductions withheld on your final paycheck.
  • Your company-provided coverage will end on your last day with Caleres. You may be able to continue supplemental coverage by completing the portability form. You also may be able to convert your basic life insurance — call Securian at 1-866-365-2374 to request a conversion form.

Update your personal information
