Losing a Loved One

No one wants to think about the death of a loved one, but it’s important to know how your benefits will be affected.

Make changes to your benefits

Be sure to make any of the following benefit changes within 30 days of the event:

  • Remove your deceased dependent from health, dental and vision benefits.
  • Change or stop contributions to your flexible spending accounts.

To request changes, go to Dayforce to complete the “Life Event Declaration Form” (Benefits > Forms):

  • Provide the required supporting documentation for your qualified event within 30 days.
  • You will receive a notification through Dayforce once your request has been approved or denied. If approved, you can make changes to your benefit elections.
  • In most cases, any changes you make are effective as of the date of the life event.

Submit a life insurance claim

If your spouse or child dies and you elected supplemental life insurance, you’ll need to claim your benefit.

Take time to grieve

If you have a death in your family, you may be able to use bereavement leave.

Update your beneficiary designations

Update your life, accidental death, or business travel accident insurance beneficiaries in Dayforce.

To update the beneficiary for your 401(k), log into Principal or call 1-800-547-7754.

Adding or losing a spouse can change your retirement savings goals. To make change to your 401(k), log into Principal or call 1-800-547-7754.
